Leak Adjustment Request Form
After a homeowner has identified and repaired a leak, a homeowner may request that the District consider a partial rebate on their bill. It is unlikely that a rebate will be approved for a leak on your side of the line.
The Leak Adjustment Request Form is required within two (2) billing cycles of when repairs were made when requesting the District consider a leak adjustment. Please read the form in its entirety, sign where required, and return to district@slud.us or present the completed form to the District office in person.
